Please complete every section thoroughly and honestly — the information you provide directly guides how we source, vet, and present candidates on your behalf. Once you submit this form, a formal Placement Agreement will be sent to you via HoneyBook for e-signature before your search begins. That signed agreement is the binding document for your placement.

The Baby Pros Collective uses this Agreement as the working framework for your entire placement search. Candidates are sourced, screened, and presented based exclusively on the criteria confirmed and signed below.

Any material change to compensation, schedule, job responsibilities, benefits, or candidate requirements after this Agreement is signed must be: (1) submitted in writing to your Baby Pros Collective representative, (2) documented in the Amendment Log, and (3) signed and initialed by the client family and a BPC representative.

Material changes may require a partial or full restart of the candidate search and will likely result in an extended placement timeline. The Baby Pros Collective is not responsible for delays, missed deadlines, or failed placements resulting from post-agreement changes to placement criteria.
